Thank you for your interest in the South West Montana Junior Golf Tour. Please complete this form to apply for the Tour. After you complete the application form you will have the opportunity to pay immediately by PayPal. Payment of $125.00 must be received in order for your application to be complete. Once you have completed this Tour Application, you (participant and parent or guardian) will be bound by the Membership Covenant. Agreement to the Membership Covenant is required in order to participate in the Tour.

By submitting this form, I confirm that I am the parent or legal guardian of the above named participant and give my permission for the above named participant to enter and participate on the SWMJGT junior golf tour. In addition, I understand that checking the appropriate boxes, entering our names in the spaces provided below and by submitting this form will act as my legal signature authorizing the above named participant. I also confirm that I have read and understand the entire Membership Covenant and agree to be bound by it's terms and conditions. I also commit to review the terms and conditions with the above named participant so that they too understand the rules and expectations of the SWMJGT junior golf tour. Lastly, I understand that we will need to pay the $125.00 Tour Fee to finalize this Tour Application form.
